I think it depends on where they are.
If they are perched on a free server, then yes, definitely... angelfire,
yahoo and geocities sites have no stability... same with some university
servers. The exception would be Kurt Keutzer's FAQs, he is a university
prof, not a student.
With domain sites, play it by ear/gut... personally, I rarely let people
mirror content off my sites... they may feel the same.

Yes. A listing of some sort, for future reference. My preference is a
work file like you sent before, but keep adding notations below links as to
what their status is.
Yes I would want a "mirrored by permission of author" statement, or some
such on the mirrored page itself.

I'm not sure I fully understand you, so let me go general, then specific.
First, there is no decision yet, about whether to put an FST frame or
banner on the library pages and leave them as they are, or whether to make
a new library template and drop the content into it without preserving the
original design of the page.
I prefer the latter, but some webmasters might disagree... preferring
the mirror to have their style preserved. We will still want to delete
angelfire spam bits, banners, etc.
I will want to credit the time machine with a link to where the page was
found, and credit the original author or location, with a javascript spam
protected email link, and a notice that the page is hosted by
fire-serpent.org.
I want the library to have a note in the footer about the condition of
the copyright and ...
well, something like this, especially for the "iffy" ones:
